Information Needed to Report an Injury
We will guide you through series of questions related to the accident, employee name and injury type, medical treatment and employer information. Be prepared to provide your Federal Employer Tax Identification Number and policy number. Even if you do not have all of the details, prompt reporting is essential to reduce your claim costs. Customer service representatives are available from 8 am to 5 pm (ET) Monday through Friday. After normal business hours, call 800-226-0666 to report your notice of injury to one of our on-call claims adjusters.
